Physical Therapy

Physical therapy is provided by NorthCrest Medical Center in partnership with Star Physical Therapy, and is located in the lower level of Medical Office Building 400. The clinical staff includes experienced physical therapists, physical therapy assistants, certified athletic trainers, occupational therapists, occupational therapy assistants. When is the outpatient center open?
The outpatient physical therapy center is open Monday through Thursday from 7:30 a.m. to 6 p.m., and Fridays from 8:00 a.m. to 4:00 p.m.

How do I schedule an appointment?
Appointments are made with the receptionist. You should check in and out with our front desk during each visit.

What do I need to know about the intake process?

During the intake process you will be asked to provide a prescription from your referring physician, a copy of your insurance care and some form of personal identification. If you injury is work related, you will need to provide us with the name, address and phone number of the person managing your workers’ compensation claim. In addition, you will be asked to complete your patient data sheet and sign a consent for care and treatment waiver. An intake questionnaire will be used by your therapist to help assess your injury and design a treatment program.

How long will my appointments be?

Depending on the type of treatment you have been referred for, your appointment will last from 45 to 90 minutes.

What should I wear for therapy?

We suggest you wear comfortable clothing suitable for exercise.

What are physical and occupational therapy?

Physical and occupational therapy are forms of rehabilitation that treat and prevent injuries and/or physical disabilities. The goals of therapy are restoration of physical strength, improved muscular coordination and pain management. Therapy frequently involves therapeutic exercise programs, massage and the use of modalities to relieve pain.

Will I always have the same therapist?

It is our policy to schedule you with the same team of therapists for each of your visits. Occasionally, a substitution must be made because of education, vacation, scheduling conflicts or illness.

Will I see my doctor when I come for therapy?

Your doctor will receive and approve your customized treatment plan. Appointments may be arranged with your doctor for periodic evaluations as you progress in your therapy.

Will my insurance cover my treatment?

Benefits for physical and occupational therapy vary widely from insurance carrier to insurance carrier. We will be happy to call and verify what services are covered in your benefits package.

How much will my treatment cost?

Your fees will vary depending upon your diagnosis and the extent of your treatment. For your convenience, we generally bill commercial insurance for you and ask that you pay only your co-pay portion on the day of treatment. We accept cash, check, MasterCard and Visa. Our office will cooperate to ensure that you receive all of your entitled benefits. Unfortunately, we cannot accept the responsibility of collecting or negotiating a settlement for your claim.
