Fountain Plaza Memorial Bricks

This honorary “Fountain Plaza” has been provided in part by the NorthCrest Medical Center Auxiliary as a place of tranquility for all visitors and in recognition of the many volunteers and individuals who contribute so much to the successful operation of NorthCrest Medical Center or to the community.

The NorthCrest Medical Center Foundation will acknowledge the permanent placement of your engraved brick “in honor” or “in memory” of your loved one by a certificate that will be mailed to your address. The cost of each 4” x 8” memorial brick paver is $50. Each brick may be engraved with up to three lines of text, with a maximum of 13 characters per line. Spaces and punctuation are considered characters. Please print clearly. Bricks may be purchased with cash, check, Visa or MasterCard.
