Case Management and Social Services

The Case Management and Social Services department serves the community and patient with social and medical issues by following the patient from admission to discharge.  We evaluate the anticipated discharge needs of the patient and arrange them in the community.  The case management department will round with your physician to identify any needs that a patient may have during the patient's stay at NorthCrest.  The clinical social workers help arrange living wills and any end of life issues.  They also will counsel with those in time of crisis.  It will be our pleasure to assist with any needs of our patients and families.
